For each feature below, information is entered into the App by you dictating (or typing) your personal information in response to the questions within the App. 

 

You can complete part of any feature and come back later to pick up where you left off.  All entries will be automatically saved.

The “magic” of the Great-Patient App is how it helps to optimize what Patients and Providers want in a Patient/Provider Partnership.

MY MEDICAL PROFILE

This is your personal medical history - your story of who you are and your health information. 

 

While this Feature will take the most time to complete, you need only to enter the information once. 

 

Going forward, you will have complete electronic access via the internet to your health information.

 

This can be on any device at any time (or you can download or print) your Medical Profile. 

 

All your health information is behind a Firewall on a secure site.

 

You alone are in control of who has access to your information.

 

Your Medical Profile should be updated on a regular basis to reflect your current health status.

 

Your answers to the questions within this Feature will help your Provider quickly understand:

  • Who you are.

  • What’s important to you.

  • What you are worried about.

  • What it is like to live with your health and medical problems.

 

This is critical information to help you co-produce your care and achieve optimal health.

 

The AI capability within the App will generate a summarized narrative for you and your Provider.

 

The report will use medical language for your Provider.

 

It will be structured to help your provider make the correct diagnosis and treatment plan.

 

You can email your Medical Profile to your Provider or print it and provide it to them at your visit.

IT IS IMPORTANT THAT YOU ALWAYS REVIEW ANY AI GENERATED NARRATIVE OR INFORMATION FOR CORRECTNESS BEFORE PROCEEDING. 

 

 YOU SHOULD  CONSULT WITH YOUR PROVIDER BEFORE CHANGING ANY PROVIDER DIRECTED CARE OR MEDICATIONS BASED ON AI GENERATED INFORMATION. 

 

YOU SHOULD NOT MAKE ANY CRITICAL DECISIONS ABOUT YOUR HEALTHCARE BASED ON AI GENERATED FEEDBACK WITHOUT FIRST CONSULTING YOUR HEALTHCARE PROVIDER.

MY NEXT VISIT (4 VISIT TYPES)

The My Next Visit feature provides four templates covering the most frequent types of Provider visits:

 

  1. A New Problem Visit with your Regular Primary Care Provider.

  2. A Chronic Problem Follow-Up Visit with your Regular Primary Care Provider. 

  3. An Annual Check-Up Visit with your Regular Primary Care Provider.

  4. A New Problem Visit with a New Clinician (i.e. Referral to a Specialist).​

 

(Additional Visit types are in development to include Emergency Department, Hospitalizations,, etc.)

 

After you select ​​the type of visit, the App will ask what questions you would like to ask at your next visit.

 

You will be able to dictate your questions into the App.

 

Your questions will be incorporated into your Next Visit Plan along with a list of recommended questions.

 

Using your Medical Profile, the Next Visit Plan will inform your Provider as to what is most important to you.

 

Your Medical Profile will help your Provider understand how living with your illness affects your life.

 

The Next Visit Plan will organize and document your symptoms; dates; health issues; health questions or concerns.

 

This will help your Provider focus on what is most important to accomplish during your visit. 

 

The recommended Guide Questions to consider asking at your Next Visit fall into four categories:

    1.  About Diagnostic Tests or Procedures and My Diagnosis.       

    2. About Recommended Treatment.

    3. About Recommended Surgery.

    4. About New Medications that are prescribed.

​

These Guide Questions can be printed and taken with you to your next visit along with your Next Visit Plan.

​

The App will provide suggested questions regarding a Chronic Problem Follow-Up or an Annual Check-Up.​

It includes other questions you have developed for your Provider.

First Steps for All Visit Types

Update your Medical Profile.

 

This step is critical because having the most recent, accurate information in your Medical Profile is essential to help your Provider get the right diagnosis and treatment.

​

For each section of your Medical Profile, you will be asked if anything new has occurred that would lead you to change your previous answers.

 

If there are changes in that section, update your answers accordingly.

​

Once your Medical Profile is updated, use the App's Ask AI functionality to learn more about your health status or medical problems.

 

AI will help create questions you would like to ask your Provider at your next visit (See the Using AI section). 

​​

For all types of visits, the App will extract data from your updated Medical Profile relevant to your visit type and your answers to questions within the Feature.

 

The App will use this information and AI to create a narrative (A Pre-Visit Plan) for your visit. It will use a standardized healthcare template preferred by Providers.

 

Having this information will allow your doctor to focus on what is most important to meet your needs.

MY HEALTHCARE CALENDAR

The My Healthcare Calendar feature provides an electronic health calendar.

 

This displays all your upcoming healthcare related activities in a single, electronically organized format.  ​

MY MEDICAL DASHBOARD

The My Medical Dashboard feature displays health data that matter to you. It could include your Provider’s recommendations or ones you choose to monitor.

 

Examples are key blood test markers or weight or exercise metrics like steps, time, etc. 


When data are entered, the App will display a graph of your data, displayed over a time frame, to help you monitor your personal health and medical problems.

MY FEEDBACK

The My Feedback feature is designed to provide actionable feedback to two groups:

 

  • Your Provider.

  • The App Developers. 

 

This feedback is critical to continue to improve what we do.
